We are committed to providing high quality, multidisciplinary care by professionals who recognize the need for comprehensive assessment of needs from both the client and professional's point of view.

 

The Agency, under the direction of the patient's physician provides the care needed to adequately maintain patients at home. In the environment of his/her home, the patient may be cared for thoroughly by a staff of skilled professionals dedicated to maximizing the "Wellness" state of the patient.

 

Each patient admitted to the Agency is evaluated by a professional nurse/case manager. The plan of care established by the case manager is evaluated by the interdisciplinary team on a regular basis, under the physician's direction. The plan of care is updated to meet the patient's changing needs.
